– *By submitting this form you verify that your dog is current on age appropriate vaccinations. This
includes any puppy attending to have had a Bordetella vaccination at least 48 hours prior to
coming to class.

– Puppy Clubs are not suitable for all dogs. Socialization Classes are designed for dogs that
are friendly to other dogs and people and to teach dogs how to play appropriately. Owners
of dogs who have shown aggression (or very intense) reactions toward people or other dogs are
encouraged to explore other options. If your dog shows aggression towards people or other dogs,
or is disruptive to the class, (s)he may be dismissed from class. If you have questions or concerns
about whether your dog is suitable for class, please contact the instructor directly prior to enrolling.
